Catholic Charities of Santa Clara County is a private, non-profit agency whose mission is to create a more just and compassionate community in which people of all cultures and beliefs can participate. Location: San Jose, CA How does it work? Day Break II is ultimately a collaboration between Catholic Charities of Santa Clara County and several other community partners that include: John XXIII Multi-Service Senior Center, O'Connor Hospital, Alzheimer's Association and Asian Pacific Islander Dementia Care Network, Council on Aging and MSSP, Family Caregiver Alliance, and the City of San Jose's Senior Companion Program. Day Break II is Santa Clara County's sole provider of comprehensive caregiver support services that are linguistically and culturally appropriate for Chinese and Vietnamese frail, low-income seniors and their family caregivers. The programming, which focuses on the clients' unique values, customs, and traditions, introduces families to services that help alleviate the demands of caregiving at home. Why is it successful? Day Break II has demonstrated great effectiveness in reducing burnout, depression, and stress-related illnesses in monolingual Asian family caregivers while simultaneously relieving isolation and depression in their dependent older loved ones.
