LINKs/ENLACE Project

LINKs/ENLACE Project was created as a best practice model for collaboration, outreach and intervention to families caring for persons with Alzheimer's disease or related disorders. The target population is Latinos residing in Hartford, CT who are frail and depend on a family member for their care.

Connecticut Community Care, Inc.

CT Community Care, Inc. (CCCI), incorporated in 1980, is a statewide, private, non-profit, care management organization. CCCI contracts with the Department of Social Services (DSS) to administer the CT Home Care Program for Elders in 3 of 5 state regions serving approximately 10,000 persons.

How does it work?

With one year of grant funding by DSS, CCCI developed a best practice model for collaboration, outreach and intervention to families caring for persons with Alzheimer's Disease or other memory loss. The project targets Latinos in the city of Harford, experiencing memory loss, dependent on a caregiver, functionally dependent and having the greatest social and economic need.

Why is it successful?

The project demonstrated that interventions do reduce stress in caregivers.
