OK- Alzheimer's Certification Information Disclosure Form for Long-Term Care Facilities (2011)

This bill directs the state department of health to incorporate information on the "Alzheimer's Disease or Related Disorders Special Care Disclosure form" about a long-term care facility's ability to provide care or treatment to people with Alzheimer's.
