FCA offers a new program for those caring for people with dementia

If you’re caring for someone with dementia, we have a new program that can help you with navigating care. FCA has partnered with Tembo Health to offer the innovative GUIDE program. 

Through the program, we’re offering comprehensive dementia care that addresses the medical needs of individuals and their families and helps with support services.

The best part is that the program is free for those with traditional Medicare. (Find more information about eligibility below.) Please note that you can continue to receive FCA services while participating in the GUIDE program.

How we help

Through the GUIDE program, we make managing dementia easier by providing support services, coordination, and clinical care.

Support: Each family is assigned a care team and a personal care navigator. We are here to be a resource to your family for any of your dementia-related questions and support. Our team provides education, tips, 1-on-1 coaching, and more to help families manage stress and maintain their quality of life.
Coordination: Lean on your care navigator to help find and coordinate care related to dementia. Your care navigator can help identify specialists or community resources, reach out for preliminary information, and help with making sure you keep appointments. We’ll do our best to get services that meet your needs, goals, and lifestyle.
Clinical Care: Your care team also includes clinical and behavioral providers who monitor your medical progression and support you and your care navigator with care plans. In times of urgency or emergency, connect to a doctor within minutes in the convenience of your home 24/7. Please note that GUIDE participants continue to keep their regular doctors and medical providers.

Who is eligible?

The one you care for is eligible if they:

Have a diagnosis of dementia
Are enrolled in traditional Medicare as primary insurance. If you’re not sure, check out the FAQs.
Are not enrolled in hospice or a PACE program (e.g., On Lok)
Do not live in a nursing home
